Job Title/Position: Home Health Behavioral Health Nurse
Reports To: Clinical Supervisor
JOB DESCRIPTION
The Home Health Behavioral Health Nurse uses the nursing process (assesses, plans, implements, evaluates) to provide patient care in the home setting. Responsible for implementation of the plan of care for patients in all developmental stages throughout the life span according to established policies, procedures, guidelines and nursing standards of care. This position is responsible for the care and case management of psychiatric patients in all stages of life in their homes. Performs other appropriate duties as assigned.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Assesses, interprets, plans, implements and evaluates primarily psychiatric patients according to the patient’s age and diagnosis.
- Effectively and efficiently manages the care of a caseload of psychiatric patients and coordinates their care with a multidisciplinary team as appropriate.
- Appropriately communicates information in accordance with agency policies and procedures and discipline specific guidelines.
- Contributes to program effectiveness.
- Organizes and performs work effectively and efficiently.
- Maintains and adjusts schedule to enhance agency performance.
- Demonstrates a daily commitment to the values of the agency.
- Demonstrates positive interpersonal relations in dealing with all members of the agency.
- Effectively demonstrates the mission, vision, and values of the Agency on a daily basis. Makes the initial evaluation/admission visit and regularly reevaluates the patient’s nursing needs.
- Provide accurate and timely documentation of patient services to reflect the plan of care within 48hrs of patient visit.
- Informs personnel of changes in the condition and needs of the patient.
- Counsels the patient and family/significant others in meeting nursing and related needs.
- Understands and adheres to established Agency policies and procedures.
- Processes orders and notifies physician of patient needs and changes in condition. Completes certification/re-certification orders and discharge summaries.
- Determines the amount and type of nursing needed by each individual patient.
- Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
QUALIFICATIONS FOR EMPLOYMENT
- Registered Nurse currently licensed in the State of California
- Minimum of 1 year recent Home Health experience
- A minimum of two (2) years psychiatric experience or certification from an accredited psychiatric program - preferred
- Must have a car with required insurance coverage and a valid California Driver’s license with no restrictions
- Satisfactory Criminal Clearance Record
- Excellent observation skills, verbal and written communication skills, problem solving and critical thinking skills along with the ability to treat everyone like family
