Parents In Community Action, Inc. (PICA) is a private, nonprofit agency designated by the federal government to operate Head Start and Early Head Start programs in Hennepin County. PICA serves over 2,500 children and families and offers a variety of programs and services.
Job Summary: The primary function of this position is to develop a relationship with families in order to assist in improving the health status. Provide training and information for them to be able to make sound decisions regarding the family’s heath and nutrition. Monitor all areas of Health and Nutrition for compliance with the Performance Standards.
Key Tasks and Responsibilities:
- Responsible for monitoring and follow up of individuals families’ health and nutrition and required up–to–date EPSDT requirements
- Responsible to ensure that all identified health needs are met
- Responsible for ensuring that children with a medically identified chronic health condition have current ICCP’s and Special Dietary Plans
- Provide support to families to ensure that all children have health insurance coverage
- Provide support to families to ensure that all children have a medical and dental home
- Work with the mediator team upon request to ensure that all children’s needs have been met
- Schedule community health educators to provide educational lectures at events and in parent places
- Recruit parents to participate in Health literacy and Family events
- Act as a liaison between PICA families and Health Care professionals when needed
- Knowledge of Hennepin County health resources
- Assist with agency events to provide health information (parent connect)
- Cross cultural competence
- Must possess strong organizational and interpersonal skills
- Valid Minnesota Drivers License, Insurance and dependable means of transportation
