Priority Healthcare Group is seeking a Central Admissions for skilled nursing facilities in Chesterton, IN.
The Central Admissions will be responsible for managing patient referrals in order to ensure a smooth transition into our skilled nursing facility and scheduling and interacting with patients via telephone, face to face, email, and performing a range of duties which includes gathering required information in preparation for the patient's on-site appointment.
Responsibilities:
- Clinical assessment of electronic medical charts from hospitals to determine if a facility can meet the clinical needs.
- Verification of benefits
- Obtain pre-authorization from the insurance company
- Communicate clinical information to the facility
- Ensure closure with each referral.
Qualifications:
- RN/LPN/LSW 2 years' experience in the nursing home industry in one of the following roles: Central Admissions, Director of Admissions, Clinical Liaison, Director of Marketing, and/or Social Services
- Covid Vaccine required unless an approved exemption is presented upon hiring
