What are the responsibilities and job description for the Patient Navigator position at Peregrine Team?
Peregrine Team is hiring for a Patient Navigator in Oakland, CA. This position is a full-time, contract to hire role with full benefits and competitive pay.
Job Duties:
Provides individualized assistance and support to patients and families. Assists patients through the health care system to ensure timely screenings, diagnosis, treatment, and supportive services that promote wellness.
Assists with tracking authorizations to resolve authorization barriers and delays; scheduling screenings and clinic appointments, and consultations with psychologists; finding primary and specialty care providers and providing referrals to providers.
Reminds patients of upcoming psychosocial appointments, follow up with patients who miss appointments, communicate with schools, and support families in advocating for school-based interventions.
Demonstrates the knowledge, skills and ability to provide safe, appropriate and effective services to children and adults with acute and chronic illness and their families/caregivers.
Under the guidance of a clinical supervisor, assists families/caregivers with accessing and providing linkages to needed services.
Helps to increase family’s abilities to advocate for needed services and supports for their medically fragile children and their family members.
Assists families in increasing their support network on behalf of the needs of their child/family member.
Works collaboratively with mental health practitioner to reach families who are difficult to engage in services.
Increases awareness about the importance of parent/professional partnerships.
Participates in outreach and community events.
Participates in related trainings and workshops.
Some travel will be required (will receive travel reimbursement).
Completes all charting requirements in a timely manner. Participates in data collection for research and service grants.
Performs other related duties as assigned or requested.
Qualifications:
- Patient Navigator or Community Health Worker (CHW) Certificate, or completion of the certificate within three months of employment, is preferred.
Proficiency with Windows-based software including Excel, Microsoft Word, and databases, locating information on the Internet, and ability and willingness to learn new software.
Shift 40 hours(5x8)
Prior experience with Electronic Medical records (EPIC) preferred.
Experience with extracting data from patient medical records and other types of databases preferred.
