Adult Hospital Liaison
Position #CL-420-3
(Martinsville, VA)

The position serves as the primary responsible liaison between Piedmont Community Services (PCS) and Southern Virginia Mental Health Institute (SVMHI), and Central State Hospital (CSH) for discharge planning, coordination, and monitoring of services for SVMHI and CSH patients and their families who reside in the Martinsville/Henry County, Franklin County, and/or Patrick County areas. This position would assist in the development of collaborative relationships with area landlords, assisted living facilities, and providers who can assist with guardianship appointment for patients who need this support. This position will serve out of the Martinsville office and would also be responsible for providing restorative services to adults in the PCS catchment area. Bachelor’s degree in human services or related field preferred with one year experience of direct care of any equivalent combination of training and experience which provides required knowledge, skills, and abilities.

Pay: $40,073.00 - $61,677.00 per year
