POSITION SUMMARY
The QIDP is responsible for the provision and documentation of services in accordance with all guidelines from the Department of Human Services (DHS) and includes training of direct service staff in accordance with DHS guidelines.

Essential Functions Statement(s)
#1 client advocate.
Establish contact with guardians and/or family members.
Responsible for completion of reviews, ISP's, ICAP's and consents.
Possess knowledge of entitlements and be a resource to clients, families and other agencies.
Must be a liaison for CST.
Complete intakes for the department.
Actively participate in the Human Rights Committee/Behavior Management Committee.
Responsible for the generation and collection of program data.
Convenes the community support team as required to revise the Individual Plan as part of the interdisciplinary process.
Informs the direct supervisor of all conditions that may prove detrimental to the clients well being.
Conducts all phases of client service in compliance with the laws on confidentiality and HIPAA.
Assess client's skills and abilities using preferred tools.
Regular, reliable, predictable attendance.
Performs all other duties as assigned by Supervisor.

SKILLS & ABILITIES
Education:
Bachelor's Degree (four-year college or technical school) Required, Field of Study: Social Services
Experience:
1 plus years of experience working directly with individuals with a developmental disability.
Computer Skills:
Streamline, Microsoft Office, Word, Excel, PowerPoint, Outlook
Certifications &
Licenses:
QIDP Certification or eligible to become a QIDP following Department of Human Services standards.
Other Requirements:
Must have and maintain a valid driver's license.
Must obtain twelve (12) CEU's annually in order to maintain QIDP certification.
Ability to use Phone, Fax, Copier, and file required.
Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
$50,000 bodily injury, each person
$100,000 bodily injury, each accident
$50,000 property damage

Pay: $21.55 - $23.80 per hour
