JOB TITLE: MDS Coordinator
DEPARTMENT: Nursing
RESPONSIBLE TO: Director of Nursing
JOB SUMMARY:
The Care Coordinator is a RN who coordinates and directs the nursing activities of a designated unit at PioneerCare and works with adult and geriatric residents. The RN Care Coordinator is responsible for the professional assessment and coordination of care needed for the residents of a neighborhood, nursing staff and overall operation of the neighborhood to ensure a high standard of quality care. Coordinates care provided by other members of the health care team, makes referrals for follow-up care and consults with physicians and others to provide individualized care. The RN Care Coordinator supports and assures the policies and procedures of PioneerCare are adhered to and is responsible and reportable to the Director of Nursing in coordinating the total care of residents.
REQUIRED EDUCATION AND EXPERIENCE:
- Graduate of an accredited school of nursing.
- Current Registered Nurse License in the State of Minnesota.
- Currently certified in CPR
- Two years of experience in Gerontological Nursing preferred.

PERFORMANCE REQUIREMENTS:
A. Major Responsibilities
- Conduct and coordinate the development and completion of the resident assessment (MDS).
- Maintain and periodically update written policies and procedures that implement MDS and care plan.
- Assist the resident in completing the care plan portion of the resident’s discharge plan.
- Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents.
- Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings.
- Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
- Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews.
- Provides education to nursing staff as needed for assessment completion, new employee orientation, and documentation standards based on the resident activities of daily living.
- Provides clinical coordinators feedback regarding assessment and care plans.
- Adheres to Facility Behavior Standards.

- May be temporarily assigned the responsibility for continuity and supervision of the nursing department and care provided.

- Must be able to set priorities while meeting needs of residents, staff, families, administration and regulatory agencies.
- Must be able to do arithmetic and basic calculations mentally; count, read in English and tell time.
- Must be able to effectively deal with difficult and emotionally upsetting circumstances, which involve the resident, and/or family and staff.
WORK ENVIRONMENT:
- Works in a clean, well-heated, ventilated and cooled environment.
- Works with several different pieces of equipment or machines including: Infusion pumps, Kangaroo pump, PAL lift, Accu-Check Glucometer, IVAC, O2 equipment & supplies, Maximist, Suction machine, Medi-lift, Wander Guard, Electronic thermometers, Scales, Clinitron and computer, etc.
C. Performs duties throughout facility as the job demands.
D. Is subject to frequent interruptions by residents, staff, and family.
E. Travels to and participates in meetings as requested and approved by the facility management and administrative personnel.
JOB RELATIONSHIPS:
A. Supervision
1. Is supervised by the Director of Nursing.
2. Cooperates with all members of the health care team in or out of the nursing department.
