Care Coordinator

This not-for-profit human services agency is angling for your skills as a Care Coordinator to help impact and transform the lives of youth who are at risk for or have current involvement with the juvenile justice system.

You will work directly with these youths to set goals, advocating for them with resources in their community, coaching and mentoring them to accomplish their ambitions.

You bring to the table a Bachelors degree in Social Work, or other Human Services-related field and a minimum of one year of case management experience, working with children and families with mental health or behavioral issues who are at risk of justice system involvement. You will demonstrate your competency in relationship management, team orientation, time management, and effective documentation skills.

Pay: $39,500.00 - $43,600.00 per year
