Pharmacist

Pride Health is hiring for a Pharmacist to support our client’s facility in Fort Wayne, IN. This is a direct hire opportunity with competitive pay, sign-on bonus, and relocation assistance available!

Position Details:

The Pharmacist will provide pharmaceutical care services to patients. The role involves rotating through clinical and distribution services, with the primary focus on clinical pharmacy services (>60% of the month). Key responsibilities include multidisciplinary rounding, medication oversight, preparation, dispensing, and providing drug-related guidance to physicians. The Pharmacist will also mentor pharmacy students and residents.

Key Responsibilities:

• Provide clinical pharmacy services including multidisciplinary rounding, dosing, and monitoring of medications.
• Prepare, dispense, and document patient medication therapy accurately.
• Advise physicians on drug usage and control.
• Monitor anticoagulants, antimicrobials (e.g., vancomycin), total parenteral nutrition, and patient-controlled analgesia.
• Mentor pharmacy students (APPE/IPPE) and pharmacy residents (PGY1/PGY2).

Qualifications:

• Education:
• Pharmacy degree from an ACPE-accredited institution.
• Experience:
• Completion of a PGY1 pharmacy residency or equivalent hospital practice experience.
• Licensure:
• Pharmacy licensure, or eligibility, in Indiana is required.

• Pay Range: $52.97 - $66.22 per hour
