Health Support Consultant
Position Summary:
The Health Support Consultant will provide assistance and resources to members in order to achieve better health outcomes.
Principle Duties and Responsibilities:
- Responsible for welcome calls to new plan members.
- Inform members of the Healthy Rewards program and encourage lifestyle behaviors for improved wellness and management.
- Discharge follow-up.
- Appointment scheduling assistance.
- Coordinate care with other health-related vendors for optimal patient care.
- Provide ongoing education and information to members as requested.
- Assist members with community resources.
- Update patient demographic information.
Education:
- Associate’s degree preferred.
Experience:
- Experience working with multi-cultural and variable socioeconomic populations highly desired.
- Experience in a customer call center preferred.
Knowledge and Skills:
- Excellent verbal and written communication skills.
- Ability to be open to new ideas, approaches and processes.
- Be able to effectively influence and encourage others.
- Anticipate, recognize and meet member needs and offer help.
- Composed, positive attitude, clear thinking and focused under pressure.
- Strong organizational and time management skills.
- Medical terminology.
