What is Remote Patient Monitoring?
Remote patient monitoring (RPM) uses digital tools to track patient health data from outside the healthcare setting. Patient data is collected from wherever the patient is located & electronically transmitted (securely) to their health care providers. RPM is helping to reduce healthcare costs by increasing in-home support, patient education, and accountability and preventing relapse or readmission to hospitals.
Our traditional healthcare model focuses on the hospitals and clinic settings. However, once a patient is discharged they are left to their own accord having to manage their own care. Abnormal trends can easily be missed until it’s very apparent where then patients need immediate attention and are re-hospitalized. We are here to prevent this unnecessary hospitalization by empowering our senior patients with the appropriate resources and tools. The prevalent and significant delays between when the symptoms first occur to the time of patient outreach are driving the sharp increase in hospital visits and major expensive treatments costing the industry $41 billion per year.
About Us:
PrimoCare is reversing this care model by bringing care teams to senior patients proactively. We help seniors get the care they need when they need it. We collect patient health data from smart devices that we provide them and use the data to predict and prevent unwarranted hospitalizations. We also provide personalized health coaching to help educate our seniors and to keep them motivated and compliant.

Duties and Responsibilities:
In this role, you will be part of a growing team working to improve the home-based healthcare experience. You will be representing PrimoCare and serving the patient's medical provider as you develop a relationship of trust with our patients.
The role of a Healthcare Coach is to:
- Outreach to prospective patients provided by the medical system/practice to accept their provider's invitation to start RPM and consent to the services. The patients you enroll remain your patients for care.
- Serve as the point of contact for patients enrolled with PrimoCare by their health care professional.
- Help patients set up their devices.
- Engage regularly with all assigned patients to encourage consistency with the treatment plan and ensure they remain within the data range set by their provider.
- Schedule regular monthly check-ins with the patients.
- Monitor patients' data.
- Educate patients through shared information.
- Inform the patients' healthcare providers electronically of monthly progress and any observations learned through the interactive communications.
- Attend virtual meetings with the PrimoCare team for information updates.
Minimum Qualifications:
- Possesses a Licensed Vocational or Practical Nursing Certification 2 years minimum of experience.
- Must be bilingual (Spanish/English).
- California & a compact state license registered as a nurse.
- Knowledgeable about chronic diseases and clinical pathways.
- Recent educator experience is a plus!
- Knowledgeable about the basics of chronic health conditions related to the devices and the operation of those devices: i.e. glucose monitor, blood pressure cuff, pulse oximeter, spirometer, scale, and more.
- Proficient with technology.
- Comfortable with navigating computer platforms and familiar with smartphone functionality.
- Comfortable with telephone outreach and engagement.
- Experienced with client interaction.
- Excellent communicator (written and verbal).
- Highly organized and efficient.
- Detail-oriented and good at multitasking.
- Team Player.
- Exhibits a strong sense of ambition, self-motivation, and self-discipline.
Preferred Additional Qualifications:
- Telemarketing or customer service experience.
- Tele-health experience.
- Strong clinical and technology experience.
- Patient Education experience.
Expectations and Compensation:
- Work from home.
- 5 -8hour shifts a week with no benefits ( independent contractor full time) ($35 /hr).
- Hours between 9:00 AM -5:30 PM.
- Mon through Friday.
- Will receive training.
- Will report to the Care Team Manager.
