- Maintain accurate and up-to-date medical records using electronic health record systems
- Utilize case management techniques to ensure continuity of care
- Monitor patient progress and make necessary adjustments to care plans
```Qualifications:```
- Previous experience in home health care or related field is preferred
- Proficient in using aseptic technique for medical procedures
- Knowledge of medication administration protocols and safety measures
- Familiarity with electronic health record systems, such as Athenahealth, for managing medical records
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to work independently and prioritize tasks in a fast-paced environment
- Experience in case management is a plus
