JOB INTRO:
We are looking for full time or part time RNs to provide skilled nursing care to the geriatric population in the comfort of their own home. We are hiring to cover Green Bay and the surrounding areas. Registered nurse must demonstrate accountability and responsibility in collaborating with the interdisciplinary team to establish and achieve patient goals and maintain high-quality patient care. RNs on our team have great work-life balance since they manage their schedule and have autonomy over their care.
JOB QUALIFICATIONS:
- Registered nurse with current licensure in Wisconsin.
- Graduate of National League for Nursing accredited school of nursing.
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Preferably have a minimum of two years experience, at least one of which is in the area of public health, home care or hospice nursing is preferred.
Skills, Knowledge, and Abilities: Adaptability, dependable, detail-oriented, compassionate.
Licensure/Certification: RN active license in WI
JOB OVERVIEW:
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Completes assessments and visit paperwork according to PRN Home Health & Therapy policy. Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members, and patient/family/caregiver.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
Supervises and provides clinical direction to LPNs to ensure quality and continuity of services provided.
Responsible for participating in on-call rotation.
