Job Summary The Reproductive and Child Health Nurse Care Manager is a key member of an integrated health care team and instrumental to its success.  The essential focus of this position is providing nurse care management to reproductive health, prenatal, postpartum, and targeted pediatric – and possibly other--patients at the Stephen Klein Wellness Center.    Essential Duties and Responsibilities General: ·       Provides nursing assessments and triage to walk-in primary care patients as well as handles in-coming “sick” calls from patients per the Stephen Klein Wellness Center triage protocol ·       Educates and counsels primary care patients and their families about issues including, but not limited to, medications, disease and disease management, stress reduction and healthier life-style choices, “normal” age-related milestones and anticipatory guidance. ·       Attains medical/symptom histories, performs medication reconciliation ·       Serves as the primary care coordinator between the patient, the patient’s medical providers, behavioral health provider, pharmacist, dentist, hospital-based service providers, financial assistance specialist, residential and/or behavioral case manager, peer specialist, visiting nursing agency, community health worker and/or other specialists. ·       Demonstrates excellence in clinical care ·       Assists with clinical procedures ·       Demonstrates excellence in care management by coordinating care with intensive emphasis during care transitions Reproductive and Child Health Nurse Care Manager: Panel Management: ·       Intensively manages the medical and social needs of an identified panel of pregnant and postpartum patients, their babies (up to 6 months- 2 years), as well as a targeted panel of high risk pediatric patients. ·       Manage SKWC’s “beta book” of patients who have inconclusive lab results for pregnancy/ectopic/miscarriage, assuming care of contacting, supporting, and scheduling these patients for all their care needs. ·       Manage SKWC’s panel of patients identified as having a history of high risk cervical cancer screening, assuming care of contacting, supporting, and scheduling these patients for their ongoing follow up need.   Clinical Duties: ·       Perform initial intake assessment on all new prenatal patients and document thoroughly in EHR. ·       Order lab and diagnostic tests as well as medications throughout the course of pregnancy (utilizing standing orders), follow up, and document results in NG. Communicate results to patient in a timely manner. ·       Serve as primary care coordinator for all lab and diagnostic tests, and specialist visits that occur throughout pregnancy. ·       Actively assist providers during prenatal visits, providing thorough prenatal care education, obtaining fetal heart tones, fundal height measurements, and office procedures as needed to assist with clinic flow. ·       Schedule nurse visits as needed for pregnant patients/infants when provider appointments are not available. ·       Triage all medical questions related to prenatal/postpartum/infant care. Take provider calls from outside facilities related to patients’ prenatal care. ·       Facilitate social supports (i.e. referrals to home visiting programs, donation programs, insurance-based support systems, transportation assistance, etc.) throughout pregnancy, postpartum, and infanthood. ·       Manage SKWC’s lactation support program; sees patient for one on one breast/chest feeding support visits to assess for effective latch and milk transfer from parent to infant, assessing the need for outside referrals as needed. Promotes breast/chest feeding awareness within the community, manages supply donations. ·       Performs family planning/options counseling in the office; nurse visits as needed for emergency contraception; manage a panel of patients at high risk for unintended pregnancy, offering outreach and support.   Centering Pregnancy and Parenting: ·       Collaborates with Reproductive and Child Health Team to initiate steps in Centering expansion and expansion of general prenatal and pediatric care services at SKWC. ·       Facilitate Centering sessions, as needed. Follow-up on clinical issues that arise in session.   Access and Quality Improvement: ·       Continually evaluate quality measures with Reproductive and Child Health Team related to reproductive, gynecologic, prenatal (including Centering Program), and early pediatric care to actively improve patient outcomes.        ·       Participates in quality improvement meetings and assists in carrying out appropriate measures in an effort to meet, maintain and improve quality of care within the clinic setting  ·      Participates in outreach activities to stay engaged with the lower North Philadelphia and Project HOME communities to continually promote awareness of and access to Reproductive/Child Health services at SKWC.   Minimum Qualifications   ·       Current RN license ·       CPR certification ·       Centering Pregnancy Certification   Preferred Qualifications ·       Experience in Women’s Health/OBGYN nursing. Prior experience as a Nurse Care Manager in a primary care setting preferred ·       BSN preferred ·       Public Health degree a plus ·       Certified Lactation consultant ·       Prior experience delivering services to patients who have experienced homelessness, mental illness, and/or substance dependence, often complicated by histories of trauma. ·       Knowledge and prior practice of trauma informed care ·       Proficiency in Microsoft Outlook, PowerPoint and Excel ·       Strong computer, organizational and administrative skills. ·       EHR experience     Project HOME is an Equal Opportunity Employer
