Job Summary
Assures that social service/discharge planning activities are integrated and coordinated within the home health team by effective communication at all levels.Develops specific, measurable, realistic goals and objectives, which enhance social service delivery.
Ability to perform a psychosocial assessment and reassessment on all patients. This includes pediatric, adolescent, geriatric and the general patient population.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a plan of care that encompasses the significant social and emotional factors related to the health problems.
Demonstrates knowledge of clinical social work and discharge planning.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.
Works independently, good organizational, writing and communication skills.
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
Communicates appropriately and clearly to physicians, staff, management team, and administrative team.
Coordinates and directs social services to ensure patients’ needs are met and hospital /home health policies are followed.
Provides appropriate resources and support to social services designee.
Demonstrates an ability to be flexible, organized and function under stressful situations.
EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES:
EDUCATIONAL REQUIREMENTS: Licensed in Social Work from a school for social work accredited by the Council on social Work Education.
LICENSURE/CERTIFICATIONS: Current driver’s license valid in the state of New Jersey. Valid certificate as Licensed Clinical Social Worker in the state of New Jersey.
PHYSICAL DEMANDS: Light when working in Agency settings. When providing services in the home setting, demands include walking to and from the care to the patient’s home; stair-climbing; stress resulting from exposure to potentially unsafe housing and neighborhoods, weather conditions and new environments. Frequent use of an automobile.
CONTACTS: Primary contact is with agency professional staff. Secondary contact with patients and families in home settings; local community agencies.
WORKING CONDITIONS: Usually good, but conditions are variable in-home settings including occasional exposure to insects and vermin.
Salary : $45,000 - $60,000
