We are seeking a skilled Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes.
Key Responsibilities
- Assess patient needs and develop individualized care plans
- Provide skilled nursing care to patients in their homes
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Monitor patient progress and adjust care plans as needed
Requirements
- Current Oregon Registered Nurse License
- 1 year of medical/surgical experience in an acute care setting
- National Provider BLS - American Heart Association certification
