About the Role
We are seeking a skilled and compassionate RN Care Coordinator to join our team at Providence Medical Group Montana. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and ensuring seamless coordination of services.
Key Responsibilities
- Collaborate with healthcare providers and other team members to develop and implement patient care plans.
- Communicate effectively with patients and their families to ensure understanding and satisfaction with care.
- Assess patient needs and identify opportunities for improvement in care coordination.
- Develop and maintain relationships with community resources and healthcare providers to ensure comprehensive care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
- Graduate of an accredited school of nursing.
- Montana Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
Preferred Qualifications
- Bachelor's Degree - Nursing
- 2 years - Nursing experience preferred.
- 2 years - Clinic Nursing experience.
