Use your excellent customer service skills to ensure that seniors receiving our care at home are well cared for and happy with our services. Preferred candidates will have experience in some aspect of the health care industry. Candidates who have been successful in positions with high level responsibilities in some form of customer service will also be considered.
Successful candidates will have either a caregiving background, PCA, HHA, CNA, or have 2 years experience in managing client relations in any industry and are willing to learn basic techniques for caring for someone at home.
This position requires a dedicated professional, committed to caring for others, enthusiastic about working with people and other professionals to provide the highest quality home care for seniors.
Care Manager Responsibilities:
- Meet with all new clients at the start of service to determine their needs and desires for service, create a plan for their care, and have them sign the service agreement.
- Communicate those needs accurately to the scheduling team who will select appropriate caregivers for the care required.
- Visit the client frequently to ensure they are happy with the care they are receiving.
- Perform visits to the client’s home to introduce new caregivers at the start of their shift and monthly home visits to all clients.
- Employ conflict resolution methods to address any client dissatisfaction and assist the team in creating a plan to resolve the issue.
- Input and create all new clients plan of care and requested schedule into our software.
- Ensure good matches between caregivers and clients by getting to know the caregivers and clients and by frequently assessing the satisfaction of the client.
- Enforce company policies when visiting caregivers at client’s homes.
- Work as a team with Care Managers to ensure quality care.
- Maintain warmth, patience, and professionalism with clients, client’s family and friends and staff of Provident Care.
- Contribute to a positive work environment.
- Be active in the hiring process by meeting and getting to know the new hires, so that once knowing them, you can make suggestions to scheduling staff as to which caregivers may be good matches with clients.
- Make sure caregivers have plans of care, shift reports, directions, and schedules for their shifts
- Once trained, may be required to perform some care to clients in the instance that the caregiver does not arrive on time.
Requirements
- Must be able to lift 50lbs, climb stairs, able to bend and stand for periods of time.
- Reliable transportation
- Must have valid driver's license with clean DMV record
- Computer experience helpful
- Pass a drug screen
- Pass a livescan background check
- Pass a TB test
- Be willing to accept afterhours communication and occasional after hours work when client needs require it.
