Our client has the largest nephrology provider platform with over 800 providers in 19 states. They serve patients using 14 years of experience managing outcomes using predictive analytics, care management tools, and the largest, live, CKD database with over 630,000 patients.

Their team is growing and they are in need of RN who will be at the forefront of this intensive, adaptive, and patient-focused model.

Role Summary:

• Out in the field completing home visits 70% ofthe time
• Being assigned as the Telephone Triage RN for the day when not performinghome visits.
• Collaborate heavily with your VBC colleagues including Social Workers, Registered Dietitians, Care Coordinators, and Navigators.
  

Requirements:

• Must have an RN Nursing Degree
• 4-6 years of related experience
• Experience working with chronically ill (CKD/ESRD) patients
• Preferably with Home care, case management, and/or hospital experience
