Psych360 is looking for an energetic, ambitious licensed practical nurse (LPN) to join our team! We are seeking an autonomous candidate that possesses valuable qualities and experience in addition to the core skill set of a practicing LPN. This is a field based position requiring someone who is comfortable with technology and Electronic Medical Records to assist primary providers in patient visit preparation.

We are looking for a candidate with a minimum of an LPN with previous experience in Long-Term Care.

The position also requires a candidate with experience and knowledge in psychiatry, common medications used in this specialty, insurance info, and patient records.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Employee must be able to relate to other people beyond giving and receiving instructions: (a) get along with co-workers or peers without exhibiting behavioral extremes; (b) perform work activities requiring negotiating, instructing, supervising, persuading or speaking with others; and (c) respond appropriately to criticism from a supervisor.

Analyzing contracted facilities’ residents’ medical records to make a determination as to which residents can benefit from/need Provider services to ensure overall health of facilities. Coordinate and meet with stake holders at facilities to provide proposed medical plans to treat residents who are not being serviced and to expand or eliminate services to residents who are being provided services as needed. Ensure the overall health of contracted facilities by developing plans to services all residents in need.

Analyzing and ensuring that full coverage and optimal patient visits are achieved through the most efficient plan of treatment.

Reviewing and ensuring requirements needed for quality patient care are secured by analyzing plan of treatment and engaging facilities to coordinate and update/revise plan of treatment if needed.

Conducting on-site visits with facilities prior to scheduled Provider visits to review charts and determine whether the patient treatment schedule should be adjusted based on patient’s medical needs.

Developing, suggesting, and formulating operating policies to ensure proper services are being provided to contracted facilities.

Provide advice to management as to the most efficient way to communicate with the contracted facilities regarding needs for Provider services to the residents.

Plan short term and long term business objectives at contracted facilities and communicate those objectives to management.

Analyzing patient medication lists in online database, EHR, to ensure accuracy to include identification of any patient medication discrepancies and evaluation of possible reasons and resolutions to the presented issues.

Maintaining patient EHR record to ensure completeness of record to include: consents for treatment, facility patient face sheets, and other clinical care documentation as appropriate. Obtaining any missing records to ensure full and proper patient treatment is achieved.

After hour on-call for emergency situations requiring patching through to the Provider and assisting with hospitalization as needed.

Represent company in, or make suggestions for, handling and resolving complaints, arbitrating disputes, or resolving grievances of patients and/or facility concerns.

Providing coordination between providers and facilities to ensure patient healthcare needs are timely addressed.

Provide clinical in-services to facilities and staff.

Expected to obtain and maintain necessary training and certificates in order to provide educational in-services.

Understands all privacy laws and policies and maintains confidentiality of clinical and sensitive administrative information.

Summary of Qualifications
Experience in providing healthcare services in skilled nursing facilities or assisted living communities.

Applicant must successfully pass required criminal background check and drug test.

Ability to document personal identity and employment eligibility within three (3) days of original appointment as a condition of employment in compliance with Immigration Reform and Control Act [I-9] requirements.
