Job Summary:
This position is responsible for arranging and providing aftercare and/or visitation services for clients and their families and acting as a resource person between families and various community/service programs. Performs all responsibilities in compliance with COA standards; state, federal and other applicable regulations, and Wernle policies and procedures.
Essential Duties and Responsibilities:
- Provides aftercare for 12 cases for six months following discharge from residential programming.
- Participates in the planning, organizing, and implementing of reintegration of client into less restrictive setting for a period of six months following discharge, as assigned by supervisor.
- Provides weekly contact with families within the first three months, and then monthly thereafter.
- Facilitate monthly child and family treatment meetings to monitor progress in services.
- Completes monthly reports with family’s progress in services.
- Monitor’s resident and family’s participation in aftercare services and the supporting documentation.
- Writes caseload reports and documents case management notes in Electronic Health System.
- Attends court hearings and provides testimony concerning resident and family’s progress and response to treatment.
- Works with aftercare providers to ensure sessions take place as scheduled and/or needed and that supporting documentation is completed.
- Coordinate’s medication management with aftercare medical professionals.
- Coordinate’s education services, including procurement of transcripts and education plans, enrollment, etc.
- Ensures residents and / or families review and signature of intake and discharge paperwork, including an explanation of rights, responsibilities, and complaints.
- Implements the decisions of managers within the chain of command.
- Provides a positive role model for the families at all times through appearance, attitude, and behavior.
- Exhibits sensitivity to the client’s cultural and socioeconomic characteristics.
- Travels to various communities to provide aftercare services and/or visitation supervision.
- Participates in the planning, organizing, and implementing of parent meetings for clients during placement and prior to discharge, as assigned by supervisor.
- Works a flexible schedule to meet the program needs.
- Maintains a positive relationship with other community service and program service providers.
- Serves as a liaison between the agency and the parents of clients by generating the required correspondence and telephone calls when necessary, both in writing and in person.
- Communicates treatment and pertinent administrative issues to Clinical Services Therapists, Supervisors, and Administration.
- Completes case management contacts in Health Electronic System (EVOLV).
- Attends Wernle home based and DCS mandatory trainings.
- Performs all other duties as assigned.
- Obtains minimum of thirty-two (32) hours of continuing education credits per year.
Education and Experience:
Possesses a Bachelor’s degree in social work, psychology, counseling or a related area of study from an accredited school. Background in health care preferred.
