Community Integration Coordinator
Wage Range: $15 -$20
Purpose: To support people with disabilities integrate in community activities
Duties and Responsibilities:
*Facilitate the full inclusion of individuals with disabilities in the recreation, volunteer, and build relationships in the community sites of their choosing.
*Promotes full engagement of each client in each activity
*Identify interest and gifts
*Assess client interest and abilities through the person-centered planning process
*Quality control of all services provided by support staff and natural supports
*Supervise, train and evaluate support staff.
*Assist support staff with development on a day-to-day basis.
*Monitor client progress through daily case notes
*Coordinate, assist and support client and staffs with seeking out community resources.
*Promote regular interaction with non-disabled community members
*Create meaningful community connections
Pre-Employment requirements:
- High School Diploma or GED
- valid driver’s license
- driving and criminal background check, CAPS background
Mandatory Reporting, CPR, First Aid, Defensive Driving, Med Qualification will be required upon first 90 days that Pueblo Community Resources will provide.
