Licensed Clinical Social Worker - PT Position; 1:00 - 5:00, Monday through Friday; Downtown location
This position qualifies for a sign-on bonus of $2,500 requiring a two (2) year commitment to PureView
General Statement
Under general direction from Behavioral Health Director this position is assigned to perform duties related to ensuring that individuals are aware of and have access to community resources to promote the greatest level of independence possible. This position provides therapeutic services to a wide range of clientele.
Duties and Responsibilities
- Completes bio-psychosocial assessments and utilizes mental health screening tools as appropriate
- Utilizes assessment and evaluation skills to determine patient needs
- Provides individual, family and group therapy, crisis intervention services and brief counseling in conjunction with medical visits
- Develops, implements, monitors and evaluates individual client care plans based on medical, mental health and social needs in partnership with patient’s care team
- Collaborates with primary care, dental and behavioral health providers to streamline patient care
- Identifies and coordinates care for high-risk clients and specific target populations
- Matches interventions and services to patient’s strengths and barriers
- Maintains confidentiality of patient information and adheres to HIPAA regulations
- Provides staff education and training in recognizing and treating mental health disorders and psychosocial problems.
- Maintains patient charts, including documentation of patient encounters and treatment plans according to required standards
- Meets provider productivity standards as set by Behavioral Health Director
- Completes peer review chart audits
- Reports instances of abuse and neglect
- Participates in PureView Health Center’s (PVHC) performance and quality improvement activities as directed
- Performs other duties as assigned
Required Qualifications – Experience & Education
The above knowledge, skills, and abilities are typically acquired through a combination of education and experience equivalent to:
- Graduation from an accredited college or university with a Master’s degree in Social Work and current licensure as a Licensed Clinical Social Worker (LCSW)
Knowledge, Skills & Abilities
- Knowledge of Federal, State, and local laws, regulations, and rules as they pertain to the position
- Knowledge of Medicaid billing procedures and documentation requirements
- Knowledge of the psychosocial aspects of community health and limited resource populations
- Knowledge of evidence-based treatments for mental health conditions
- Knowledge of PureView Health Center policies and procedures
- Knowledge of community resources and service organizations
- Skill in implementing the principles and practices of the professional field of social work and case management
- Skill in conducting biopsychosocial interviews and assessments
- Skill in establishing and maintaining effective working relationships with patients and staff
- Skill in resolving interpersonal conflict in a straightforward and timely manner
- Skill in establishing appropriate professional boundaries according to professional ethical standards
- Ability to work effectively in a team and share responsibilities and duties
- Ability to set priorities, be organized and take initiative

Physical Demands
Duties are generally performed in an office setting where hazards and discomforts are controlled and modifiable. Position requires knowledge and use of typical office equipment including telephone and personal computer. Must be able to move or lift materials weighing up to 30 pounds. Position requires frequent contact with fellow employees, patients, and citizens.
