AGENCY SUPERVISOR
TITLE OF IMMEDIATE SUPERVISOR: ADMINISTRATOR

DUTIES
The entire clinical program shall be under the direction of the agency supervisor. The agency shall organize the personnel and clinical activities of the home health agency so that safe and adequate care will be provided to the patient.
The skilled nursing service of a home health agency shall be under the direction of the agency supervisor.
The Agency Supervisor, in accordance with policies and standards established by the
Governing Body, and under the direct supervision of the Administrator, assists with the planning, coordination, leading, controlling and evaluation of home health services.
The Agency Supervisor is responsible for coordination and liaison with appropriate parent and affiliate Departments and committees, development of policies and guidelines for and obtaining input from service personnel and providing professional input to related administrative systems.
The Agency Supervisor directs and supervises professional and auxiliary personnel rendering client care services, performs marketing duties and fosters public relations for the company, consults and plans with health care facilities regarding staffing needs, and communicates the concept of quality client care to the general community.
The Agency Supervisor interprets service needs to the Administrator and acts as a client services administrative liaison.
Remains available during all business hours to participate in activities relevant to professional services rendered.
RESPONSIBILITIES
Supervises all RNs, licensed practical nurses, home health aides, therapists, social workers and other clinical personnel employed by the agency or with whom the agency contracts for services

Develops standards that ensure safe and effective services to clients and families.
Assures that all staff providing patient care maintain the professional standards of community nursing practice
Maintains and adheres to agency procedure and patient care policy manuals
Participating in establishing service policies and procedures
Participating in selecting and evaluating nursing personnel and other staff providing patient car
Coordinates patient care services
Makes patient and personnel assignments; Keeping and maintaining records of case assignments and case management
Preparing and maintaining the schedule of cases to be brought to the clinical record review committee
Assists in Agency evaluation activities; conducts selective program evaluations to improve deficient services and developing and implementing plans of correction.
Evaluates the program and services
Updates procedures and job descriptions as policies and procedures change.
Coordinates with Administrator planning activities to develop administrative service policies, procedures and practices for the Agency.
Participates with the Administrator and Executive Director in the formulation of a business plan, i.e., strategic goals, resource allocation plan(s) and a yearly budget.

Consults, plans and works with Administrator in service policy formulation and programmatic development and implementation including assessing adequate and appropriate staffing.
Coordinates referrals

Coordinates with other program areas and senior management as appropriate according to the organizational structure.
Acts as liaison with Staff Development for program areas.
Assesses appropriate staff supervision during all service hours.

Collaborates in the development of an effective and efficient client care documentation system including statistical compilation and analysis relative to cost and staff service delivery.
Participates in selected orientation programs.

Submits formal written proposals to Administrator for all new programs or services (with complete cost/benefit analysis).

Participates with other health, civic, education and professional groups as directed by the

PROMOTES STAFF DEVELOPMENT

Encourages and assists personnel to continue self-improvement through formal education, educational meetings and active participation in professional and related organizations.
Assists with interviews and hires select home health care personnel.
JOB CONDITIONS
1. Position is stressful in terms of meeting deadlines.
2. It requires minimal lifting of office records and printouts.
3. It is primarily a desk job which essentially involves sitting, standing stooping and walking, as well as an inordinate amount of telephone communication.
4. Travel is required, by car or airplane to local, out-of-town or state seminars, conferences or meetings.
5. Must be able to read 12 point and larger type.
6. The ability to communicate well, both verbally and in writing is required.
EQUIPMENT OPERATION
The job requires the ability to utilize a PC, calculator, multi-line telephone, and other related office equipment.

COMPANY INFORMATION
Has access to all client medical records and client financial accounts, personnel records and company financial records, which may be discussed with all management staff.
No one person may hold the positions of both home health agency administrator and agency supervisor.
If the licensed home health agency is also licensed as a home nursing agency, the agency supervisor may supervise the provision of skilled nursing services in the home nursing agency only if there are equally qualified individuals available in each licensed component of the organization to act in his or her absence.
QUALIFICATIONS
1. A Registered Nurse licensed in Illinois, who is a BSN (from a program approved by the National League for Nursing) and has at least one year of nursing experience.
2. If the RN does not have a BSN, they must have three years of nursing experience that meets the following requirements:: a. At least two years of the nursing experience must have been in a home health agency, a community health program that included care of the sick; or a generalized family-centered nursing program in a community health agency and, b. At least two years of the three years nursing experience must have been obtained within five years prior to current employment with the home health agency.
3. Must have a criminal check.
4. Must have a current CPR certification.
