Position Summary:
Manages client care, supervises and coordinates home health care, and adheres to state and federal regulations pertaining to job requirements. Ensures quality and safe delivery of nursing care services and fulfills additional performance responsibilities as assigned and as necessary.
Minimum Qualifications:
Education Experience
- BS in Social Work
- 1 - 2 years experience in a related field: hospital, nursing home, home health care, etc., preferred
Physical
Visual/hearing ability sufficient to comprehend written/verbal communication- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
- Ability to deal effectively with stress
Equipment Used
- Familiar with and knowledgeable in the use of various types of equipment used in the nursing and health-related fields
Main Responsibilities:
Perform/complete all required documentation in a timely manner according to specific deadlines- Ensure quality and safe delivery of home health care and nursing care services
- Develop, implement and evaluate individual home health client care plans using agency policy and procedures
- Develop written staff assignments
- Complete clinical documentation as required; submit documentation to the Clinical Director
- Conduct client case conferences
- Communicate with clients and family members regarding client home health care and plans of care
- Conduct supervision of Attendants
- Act as a liaison with assigned clients
- Coordinate purchases of supplies and equipment with clients and family
- Make direct recommendations regarding care to Clinical Director
Main Responsibilities (continued):
Ensure that all state, federal and local rules and regulations are adhered to- Stay updated on all Health Care related practices and issues
- Maintain consistent and concise lines of authority and responsibility
- Promote education for home health care staff and clients, client families, and the community
- Demonstrate participation in meeting requirements for state health department surveys of home health care agency
- Be available for on-call duty as necessary
- Complete other job-related duties as needed
