What are the responsibilities and job description for the Registered Nurse (RN) ACTT Team position at Pyramid Healthcare Inc?
Our Assertive Community Treatment Teams (ACTT) provide a “hospital without walls” program that brings high intensity behavioral health services into the community to serve people who may not have access to the care they need.
ACTT is a comprehensive team of professionals from many disciplines that provide tailored services to meet the client’s mental health, substance use, vocational and housing needs. ACTT improves outcomes for people with severe mental illness who are most at risk of psychiatric crisis and/or affected by other social determinants of health.
This position has a flexible schedule of 5 day 8 hour day shifts or working 4 day 10 hour shifts
Duties:
- Provide direct patient care, including assessment, planning, implementation, and evaluation of nursing care plans
- Perform case management duties to ensure continuity of care for patients
- Conduct diagnostic evaluations and interpret results to determine appropriate interventions
- Provide specialized care for patients with dementia or other cognitive impairments
- Administer medications and treatments as prescribed by physicians
- Utilize electronic health record systems for documentation and communication purposes
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
Qualifications:
- Valid Registered Nurse license in the state of [location]
- Bachelor's degree in Nursing (BSN) preferred
- Minimum of 2-3 years of experience with persistent mental illness
- Strong assessment and critical thinking skills

- Must have at least 2 years of experience working with individuals with severe and persistent mental illness. Prefer RN status or be an Advanced Practice Nurse (APN)
