Job Description:
A Case Manager/ Support Coordinator’s duty is to support individuals that have a developmental disability to lead a meaningful life in the community of their choosing.
Each Case Manager will be responsible for carrying a caseload of 40-60 participants.
Driving is required for this position, as the Case Manager will be required to visit Participants in their homes.
In addition to meeting with participant’s in their homes, the Case Manager will be required to maintain each caseload participant’s personal file.
This position requires significant time spent on documentation (as required by the parish and state). Case Managers hours are M-F 8:30 to 4:30, with occasional after hours appointments.
Requirements:
Applicants are required to have earned a Bachelor’s Degree in a human service related field such as psychology, criminal justice, education, social work, or sociology.
Applicants must have a vehicle and valid Louisiana car insurance.
