Job Title: Behavioral Health Consultant

Duties:
- The Behavioral Health Consultant will focus on providing individual and group counseling, comprehensive assessments, and crisis interventions for patients with multiple chronic medical conditions, as well as providing advice and support to the multidisciplinary team about behavioral health perspectives, to assist in improving patient care.
- The Behavioral Health Consultant will also be responsible for supporting our patients in improving their knowledge and awareness about the impact of their chronic health conditions on their mental health and wellness, including the impact of tobacco use, Drug/Alcohol use, and High Risk Sexual Behavior.
- The Behavioral Health Consultant will work collaboratively with our other professionals to develop a strong care plan.
- This role is centered in a trauma informed care approach with emphasis on harm reduction and motivational interviewing to elicit behavioral change towards adherence with medical recommendations and treatment plans.
- This role serves as the behavioral health subject matter expert (SME) within an interdisciplinary care team comprised of primary care providers, nurses, medical/social work care managers, nutritionists, psychiatrists, pharmacy, community health workers, and social determinants of health (SDoH) specialists.
- The BHC makes referrals for behavioral health levels of care that are not provided through providers, e.g., ACT, IOP, PHP, residential treatment, inpatient services.
Qualifications:
- Clinically licensed in the state of practice with 3 years of experience in a behavioral health related field including social work, mental health, and/or addiction.
- Qualified clinical supervisors will be given preference.
- 2 years of experience in a community mental health center, managed care organization, FQHC, inpatient psychiatric facility or other setting that serves clientele with complex behavioral health needs.
- Experience with complex government-sponsored populations preferred, e.g., Medicaid, Medicare beneficiaries.
- Experience with EMR and efficiency in completing EMR notes within 48 hours
Shift:
M-F 8am-5pm w/1 hour lunch.

Pay: $80,000.00 - $84,000.00 per year
