Case Manager
Position Summary:
Under the supervision of the Director of Unsheltered Programs, in partnership with the Unsheltered Clinical Director Unsheltered Outreach Coordinator, the Case Manager works with the Unsheltered Outreach Task Force to build relationships with unsheltered neighbors, provide advocacy, and assist with connection to benefits and housing.
The primary goal is to identify unsheltered neighbors through direct street outreach activities and through community referrals from mental health, hospitals, local law enforcement and municipalities and other social service agencies. In identifying highly vulnerable unsheltered neighbors, the Case Manager will administer assessment tools, facilitate placement into emergency housing, if needed, and connect to social services and permanent housing. The Case manager will also assist neighbors in obtaining housing readiness documentation and accompany them through housing application process. The Case Manager will work as part of a larger team within the Western Slope Region within Colorado service planning area.
Primary Responsibilities
Work collaboratively with the Unsheltered Outreach Coordinator to conduct outreach in the community, focusing on those who are most vulnerable. Outreach will occur in identified “hot spot” neighborhoods at times when individuals are most likely to be found (including early morning and evenings).
Complete a Vulnerability Index Service Prioritization and Decision Assistance Tool (VI-SPDAT) to determine vulnerability and acuity of neighbors during street outreach as well as other outreach and referral processes.
Complete a HMIS intake, HMIS Release of Information and give this information to the HMIS data entry team member.
Assist unsheltered neighbors with accessing resources and making referrals; continue to engage with unsheltered neighbors until shelter or permanent housing is obtained.
Provide advocacy for unsheltered neighbors when they encounter barriers.
Assist in obtaining housing readiness documentation such as ID, social security card and income verification and work with Unsheltered Outreach Navigator to provide transportation to appointments as necessary.
Work with Case managers from other agencies in the region to provide team outreach services.
Assist in identifying appropriate housing and completing housing applications.
Provide support for people as they transition from homelessness to housing.
Maintain complete client records, daily activity logs, mileage logs, and other reports as directed.
Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other local officials), and providers of services and resources to unsheltered neighbors.
Attend team meetings, case conferences, training workshops and community meetings as needed.
Participate and lead the unsheltered task force

Qualifications
Has a BA in social work or related field or 2 years of equivalent social services experience.
Exhibits dedication to the mission and vision of Recovery Resources
Works effectively with people of varying racial, ethnic, cultural, educational and socio-economic backgrounds.
Has basic understanding of severe mental health, physical health, and substance abuse issues and symptoms.
Listens with sensitivity to other people’s feelings, needs, and point of view; demonstrates tact and courtesy in expressing options or ideas, and recognizing opportunities to enhance community relations. Is proficient with computers and related technology; must maintain neighbor-related data tracking systems, including case notes.

Accountability and Review:
3-month probationary period with a performance review, followed by annual evaluations
