How you make a difference
The Social Services Continuity of Care Coordinator plays a pivotal role in assessing, treating, and coordinating discharge activities for forensic individuals served within the hospital, facilitating seamless transitions between hospital care and community support systems. This position serves as a liaison between the hospital, courts, community forensic teams, case managers, and other stakeholders, ensuring effective communication and collaboration throughout the continuum of care. By managing discharge processes and fostering relationships with external partners, the Coordinator contributes to the successful reintegration of forensic individuals into the community while promoting continuity of care and support.
Key Responsibilities
- Coordinates forensic and discharge activities, including visits to aftercare facilities and engagement with legal and community stakeholders, to ensure adherence to discharge plans.
- Develops and maintains effective relationships with legal professionals, community case managers, and mental health center staff, attending relevant meetings to advocate for persons served.
- Conducts professional assessments of community placement facilities and accompanies persons served to court, ensuring compliance with court requirements.
- Maintains accurate medical records in compliance with hospital and governmental regulations, participating in performance improvement activities to enhance service delivery.
- Continues professional development to stay abreast of legal legislation and cultural diversity issues, providing individualized care and treatment to forensic persons served while collaborating with colleagues for their well-being.
Qualifications & Requirements
Education
- Bachelor’s Degree in social work, criminal justice, or mental health counseling is required.
Experience
- Two (2) years of experience dealing with severe and persistent mentally ill patients, is necessary. Additionally, three (3) years experience working with the court systems and personnel system, preferred.
