JOB SUMMARY AND ESSENTIAL FUNCTIONS As a part of the Care Coordination and Management team, responsible for facilitating care coordination services for patients in need of enhanced services. Eligible patients will be ensured that they will receive timely, high quality and efficient health care and support services through the development of a multi-disciplinary care plan, self-management goals and referrals to both internal and external resources.
JOB REQUIREMENTS
EDUCATION AND EXPERIENCE:
- Completion of a RN, LPN or Certified Medical Assistant program.
- Twelve (12) months experience in health care setting working with patients with chronic conditions, Prenatal/Perinatal patients, Children & Families
PREFERRED QUALIFICATIONS: - Two (2) years’ experience with care coordination/care management in a medical setting
- Prior experience with managing competing priorities
- Medical terminology
- Bi-lingual skills
- Experienced with public speaking, group training
- Experience in Project Coordination

LICENSE OR CERTIFICATION: Registered Nurse, Licensed Practical Nurse OR Certified Medical Assistant licensure to work in the state of Tennessee. CPR/BLS certifications are required
PHYSICAL DEMANDS: Frequent walking, driving and sitting; speaking, hearing and visual acuity is needed; local travel is required; occasional out-of-town travel is required
