Service Coordinator - Supportive Housing - 1 (SH1) - $17.25/Hr

FT 40Hr/wkAssists those who were previously homeless in securing and maintaining housing and links program participants with services, resources, and entitlements. Collaborates with landlords and develops service plans to improve quality of life. Responsibilities include: goal planning and progress reporting, and activities promoting skill development required to enhance and maintain healthy living within the community (examples include making and keeping important appointments with healthcare providers, transportation services, budgeting, social skill development, employment readiness, and crisis intervention). Documentation of services in the Electronic Medical Records System (EMRS).

Hours:Monday - Friday, 8:00 am - 4:00 pm

Duties and Responsibilities:

  • Implements program and agency policy and procedures
  • Participates in the service plan process including creating initial IRP, crisis plan, and risk assessment, in collaboration with Program Director and Clinician as needed
  • Attends team meetings as designated by the Program Director
  • Provides rehabilitation, basic support, case management, and crisis interventions
  • Provides required documentation of services within the deadlines of the Agency
  • Reports on residential status to Program Director and staff
  • Provides input for team decisions regarding admission, discharge, and counselor assignments
  • Assists program participants in maintaining apartments and landlord standards when applicable
  • Assists program participants in financial matters as needed
  • Provides transportation related to services when necessary
  • Maintains CPR/FA/AED certifications
  • Monitors program participant’s compliance to conditional release order when applicable
  • Provides coverage as needed at any of the Agency sites per Agency need
  • Engages in continuous efforts to access, obtain, and respond to pertinent agency information and communications and disseminates as appropriate
  • Provides conditions that maximize safety for co-workers and program participants
  • Other responsibilities of Reliance Health staff, such as participation in Agency events, adhering to Agency Policies, training requirements etc.
  • Perform duties while demonstrating Agency Core Competencies and values

Education and Experience:

  • Preference will be given to persons with lived experience
  • Bachelor’s Degree in Human Services or equivalent work experience in the provision of mental health
  • Work experience in the provision of mental health services preferred
  • Valid CT Driver’s License, vehicle registration and insurance
  • Good driving record
