Job Title: Behavioral Health Care Manager

Department: Mental Health Care

Location: Remote

Duties and Responsibilities:

• Perform behavioral health assessment and diagnostic evaluation
• Provide evidence-based behavioral interventions for patients
• Develop behavioral health treatment plans under direct supervision of psychiatric consultants
• Provide clinical and task supervision for behavioral health care coordinator
• Coordinate care for patients in the interdisciplinary team including primary care providers and psychiatric consultants
• Monitor and tract treatment engagement and outcomes
• Provide crisis intervention as needed
• Coordinate supportive care with family, community organizations or other agencies as needed
• Maintain accurate records in EMR system
• Attend case conferences, program meetings, and trainings as needed

Requirements:

• Valid LMSW/ LMHC license in NY state required
• 1 years of experience providing evidence- based behavioral interventions in mental health clinic, preferred
• Strong communication and written skills
• Ability to work in fast paced environment
• Knowledge of and experience in EMR system.
• Bilingual English and Chinese (Mandarin or Cantonese) required
