Overview:
Develops/implements care plans for persons served in area of responsibility. Assesses health status, identifies problems and needs of person(s) served in their assigned caseload, develops and implements nursing care plans in collaboration with programmatic or behavioral health staff, and oversees medical record maintenance. Administers direct nursing care for persons served. Advises persons served, their guardians and staff on health maintenance and disease prevention. Supervises LPN’s, CNA’s and DSP’s related to health services activities and as directed
Responsibilities:
- Participates in admission decisions, ensuring that company admission criteria are met, and the needs of the person can be met relative to staffing and competency of direct service staff.
- Conducts assessment and reassessment of persons served
- Develops/updates care plans. Determines persons served needs and secures physicians orders as needed.
- Coordinates communication between team members/attending physicians/and all direct caregivers to ensure the appropriateness of care and outcome planning.
- Ensures compliance with state/local/payer requirements through review of documentation, attendance at IDT and care coordination activities.
- Ensures that caregiver clinical documentation meets company and payer standards.
- Participates in the recruitment/interviewing/selection/orientation of LPN/LVN’s, direct care/service staff; provides supervision and evaluates their performance
- Provides coaching and education related to the health and well-being of persons served.
- Participates in performance improvement activities, maintains ongoing clinical knowledge through internal/external training programs, provides interpretation of knowledge and direction to staff.
- Addresses acute and on-going health issues
- Ensures that medical appointments are scheduled. May attend medical appointments when indicated based on the individual’s health needs and condition
- Ensure physicians orders and plan of care meets the needs of the individual served and are implemented by direct care staff (or provides directly).
- Secures physicians orders for needed medications/treatments
- Ensures that new/changed medication/treatment orders are transcribed onto the MAR/TAR and EHR as indicated
- Prepare and present in-service to staff (when indicated)
- Document above actions in Nurse’s Notes or Service Delivery Log
- Keep QIDP/PM informed of health needs and of the need to have an IDT meeting
- Supervise the health care team (LPN, and health & wellness services provided by CNA, DSP staff)
- Review MARs - investigate and resolve any medication errors, missing doses, etc
- Ensure that complete mediation counts occur in each home reconcile counts to ensure that medications were given as ordered, reorder and restock medication supplies.
- Monitor documentation, review clinical information (e.g., MARs, weights, vital signs, etc.) and follow-up as indicated
- Ensure MARs are prepared for upcoming month. (MARs must be reconciled and placed at each group home prior to the last day of the month.) For individuals to receive medications while attending day programs, ensure a MAR is present and accurate at the Day Program

- Maintains certification an any/all state-required clinical certifications (e.g., Medication Delegation instructor)
- As new physician orders are received, ensure the order is transcribed into the record, and that medication orders are transmitted to the Pharmacy.
- Complete Quarterly/Monthly Nursing Review which includes an assessment for each individual
- Complete AIMS assessment (if not completed by psychiatrist)
- Attend Psychiatric appointments as indicated
- Ensure clinical/medical records are up to date, including medication, clinical histories, Seizure Records, weights, immunizations, and medical appointments, etc. Review all available data and reporting, identify areas in need of attention, contact physician(s) and make revisions to the plan of care/service as applicable
- May delegate tasks to other direct-care/support staff as applicable and in compliance with company, State and payer requirements
- Ensure all required screenings are completed as applicable (e.g., Fall Risk, Pneumonia, Mammograms, etc)
- Participate in on-call duties to ensure weekend and after-hours access to clinicians by staff and individuals served (participates in on-call rotation as assigned)
- Actively participate in applicable meetings and committees (e.g., IDT, HRC, Safety/Quality committee, and annual person served directed/active treatment plan meetings
- Other duties as assigned and within scope of practice
Qualifications:
- Must have at least one year of nursing experience, including 6 months in a supervisory role
- Clinical experience in providing services and supports to individuals with intellectual and developmental disabilities or related disorders preferred
Education:
- Degree/ certification from an accredited school of nursing.
Certificates, Licenses, Registrations:
- Must maintain licensure as a Registered Nurse (RN) in the state of practice, which is current and remains in good standing
- Valid driver’s license from state of residence
- CPR Certification
