Revitalized Specialty Infusion Services is a leading provider of Home Infusion Nursing Support in Massachusetts, Connecticut, Rhode Island, and Southern New Hampshire. Revitalized partners with local and nationwide Home Infusion Pharmacies to meet the skilled infusion nursing demands of a rapidly growing infusion industry.
RN to perform high-tech skilled nursing infusion services and clinical tasks associated with patient infusion home care including: assessment, obtaining IV access, education, and infusion, and coordination of care.

Essential Duties and Responsibilities (not limited to):
- Coordinates and schedules care with their list of managed patients along with other clerical duties to confirm their patient visits.
- Tracks patient supply needs and orders them based on pharmacy protocol.
- Prepares/compounds sterile admixtures per MD orders for immediate use when required.
- Completes and submits nursing and progress visit notes including admission, plan of treatments, and assessments per policy same visit day.
- Ensures proper labeling of medications in the home.
- Manages emergency supply kit, including monthly expiration checks and orders them as needed.
- Completes full medication profile review at every visit.
- Ensures a clean environment for infusion preparation and administration in the home.
- Communicates with Revitalized, pharmacy, MD office, and referral sources to solve patient care issues.
- Lab draw and drop off at local hospitals/clinics per MD orders.
- Updates schedule system and follows workflow protocols.
- Communicates any schedule conflicts in advanced notice.
- Commits to agreed upon in the signed Nursing Service Agreement.
- Attentively follows Revitalized LLC Policy and Procedure Manual and Employee Handbook.
- Remains compliant and follows guidelines found in Revitalized Education policies/skills checklist.
- Keeps nursing folder up to date with all requested credentials and licensure.
- Attends 1 hour virtual class before onsite training visit with nurse supervisor.
Qualification Requirements:
- Expectations of RN: clinical excellence, skilled, reliable, responsive, personal, professional, good communication skills, willing to listen and follow directions, and receptive to constructive criticism.
- Provide the highest level of professionalism, responsiveness, and communication in order to build and maintain the maximum customer base possible.
- General Infusion Experience required. Speciality training provided.
- Strong IV Skills required.
- Port Experience preferred.
- Valid driver's license, maintain automobile insurance coverage, and personal professional liability insurance, and have access to a reliable automobile: required.
- Ability to communicate effectively both verbally and in writing.
- Possess good organizational and time-management skills.
- Dependable in attendance and job performance.
- RN Nursing license active in State of care required.
- BLS Certification required.
- Hep B, PPD/TB Risk Assessment, proof of immunizations required.
- Minimum one (1) to two (2) years acute nursing and IV insertion experience required.
- CRNI, IGCN, or other IV certifications preferred.
- Background Check consent required/Drug test per state requirement.
- Covid vaccination preferred, but not required per each State’s regulations.
- Two (2) References required from most recent or relevant job experience.
Reports To: President, Director of Nursing, Account Manager(s), Nurse Schedule Manager, Nurse Supervisor, Schedule Manager, and Client Relations Manager.
