Psychiatric Intake Coordinator is responsible for coordinating all psychiatric intake referrals. Psychiatric Intake Coordinator is also responsible for completing psychiatric intakes for clients referred to receive medication management services. Psychiatric Intake Coordinator works with healthcare professionals like doctors, nurses, and case managers to determine if client is appropriate to receive psychiatric services at Josselyn. The Psychiatric Intake Coordinator is also responsible for duties assigned by the RN Psychiatric Services Manager.
Psychiatric Intake Coordinator will complete psychiatric intakes, obtain all required documents from clients, and schedule client’s psychiatric evaluations and maintain spreadsheet, tracking outreach attempts and outcomes of all referrals. Psychiatric Intake Coordinator will provide appropriate linkage and referrals to clients in need of a different level of care for services and support in the community.
Essential Responsibilities and Metrics
• Coordinate all psychiatric intake referrals • Screen referrals to ensure criteria is met for medication management services • Complete Psychiatric Intakes in a timely manner • Facilitate client signatures of all requested documents • Schedule client’s psychiatric evaluations • Maintain spreadsheet, tracking outreach attempts and outcomes • Facilitate client referrals for additional services consistent with assessed needs for appropriate level of care • Communicate with the Clinical Team regarding wait time • Regularly consult with psychiatric providers and/or RN Psychiatric Services Manager on specific client related clinical issues • Complete all clinical documentation and required reports in an accurate and timely manner • Complete Service Ticket/Progress Note within three working days of day of service • Assumes leadership role either by own initiative or when asked by RN Psychiatric Services Manager • Receive input from the RN Psychiatric Services Manager and provide feedback regarding administrative issues • Refer client to RN Psychiatric Services Manager when appropriate • Participate in supervision with RN Psychiatric Services Manager • Other duties as assigned by RN Psychiatric Services Manager • Participate in Staff Development seminars • Attend weekly case management meetings • Clear vacation and time off request with RN Psychiatric Services Manager • Appropriate and timely response to RN Psychiatric Services Manager requests for data or special projects • Participate in orientations and Josselyn training programs • Protect client confidentiality and abide by the ethical statement of the agency • Maintain Productivity goals as determined by supervisor
