LEAD CARE ASSSOCIATE JOB

The Lead Care Associate is responsible for helping to implement the client care plan after it is developed by the care manager creating extraordinary client experiences; and communicating with care staff, client, families, care partners and referral sources. Plays an integral role in interfacing with care staff, clients and families to ensure safe, appropriate, and meaningful care is provided. Serves as a mentor to caregivers and supports their care delivery. Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment

ESSENTIAL DUTIES AND RESPONSIBLITIES:

• Conducts client introductory and supplemental visits on an as-needed basis.

• Identifies and reports opportunities to improve the client experience.

• Minimizes caregiver turnover through effective mentoring, relationship building and communication skillswith assigned caregivers.

• Helps clients and families understand changes in the plan of care.

• Assist in facilitating a good client and caregiver match.

• Will personally fill shifts as needed for successful client care when emergency care, fill in, and caregiver call offs result in a need.

• Must be able to adapt to an ever changing schedule as the “go to” person for fast reaction and emergencies, as it relates to client care.

• Maintains complete, accurate and timely client records in ClearCare.

• Exhibits enthusiasm, charisma, excitement and a positive “can do” attitude toward creating an extraordinary client experience.

• Be available for on-call duty outside of normal office hours, as needed.

• Other duties as assigned by the Agency Directors.

• Reports to shift within one hour of being dispatched.

• Must maintain a flex schedule

QUALIFICATION: 

  • Must have a current CNA/HCA certificate 
  • Valid drivers license/insurance and reliable transportation 
  • 2 years caregiving experience 
  • Must be able to lift/transfer without weight limitations
