About River Edge Behavioral Health
Serving more than 10,000 Georgians, River Edge continues to be one of the state's leading resources for mental health, substance use disorder treatments and supports as well as individuals with intellectual and developmental disabilities. The full continuum of River Edge Behavioral Health services provides state-of-the-art treatment and support services for more than 13,000 children, youth, families, and adults annually, who experience mental illness, addiction, co-occurring disorders or developmental disabilities.
We are searching for a Case Manager to join our Core Team at our Bibb County location. This position is a full time role.
Duties and Responsibilities:
- Implements and organizes the delivery of specific social services within the community
- Monitors patient's/client's/consumer's progress toward treatment goals
- Completes patient/client/consumer intakes based on interviews with patients/client/consumer, their families, significant others and appropriate community agencies
- Assist clients in locating and utilizing community resources including legal, medical, financial assistance and other referral services
- Provides short-term case management and referral services to clients with emergency situations
- Provides crisis intervention and emergency shelter services
- Counsels clients living in group homes and halfway houses; assists in pre-release and release planning. Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
- Implements life skills workshops and programs in behavior management, youth services, community and social services
- Maintains program statistics for purposes of evaluation and research
- Prepares intake reports and case history records
- Other duties as assigned
Qualifications Needed:
- COVID Vaccinated
- Bachelor's degree in social work or related field is preferred
- At least one year of experience in social service delivery in lieu of education
- Must be experienced with Microsoft Office Suite and also be able to learn other relevant software applications including electronic medical records
