Summary
The Outpatient Orthopedic Clinical Nurse Coordinator will manage all clinical aspects of the orthopedic service line. The clinical coordinator will serve as the clinical expert in the delivery and management of comprehensive care while utilizing research and evidence-base practice. The coordinator is the liaison between all stakeholders that directly or indirectly interact with patients. (Including but not limited to surgeons, primary care, nurses, physical therapists, case management, quality committees, marketing, rehabilitations units, and community resources.) The coordinator will exhibit enthusiasm towards adopting a systems-based approach and demonstrate leadership while fostering a culture of team values to improve patient outcomes and safety.

Experience/Education Requirements
- Graduate of an approved school of nursing, BSN/MSN preferred.
- 2-4 years of clinical experience in Orthopedics.
- Demonstrates operational leadership experience including an understanding of core measures and CMS regulations.
- EPIC documentation experience preferred
- Current Illinois RN license.
- ACLS certification required.
- Orthopedics Nurse Certified (ONC) preferred.
- Plans, coordinates, and directs all activities between the interdisciplinary team to direct and facilitate departmental throughput to attain positive outcomes.
- Coordinates with ancillary departments such as lab, radiology, rehabilitation, home health, DME for safe compassionate care to ensure positive outcomes for patients.
- Acts as a liaison to departmental staff, medical personnel, patients, and family members. Assists in any patient care concerns, issues, and complaints.
- Performs educational meetings to keep staff updated and informed with current evidence-base practices and changes to ensure the best patient outcomes.
- Serves as mentor and role model for staff development and a resource for the team. Insures staff meet all competency and employee health standards as well as mandatory education standards. Provides input into and assists in the completion of the annual performance review of those staff members.
- Supervises team members activities and provides input on chart development, orientation, and training.
- Ensures safe and cost-effective staffing levels as well as ensuring staff follow proper safety and nursing care standards.
- Ensures that staff act professionally, are courteous, and respectful towards patients, physicians, guests, and staff. Considers the diverse needs of each patient while ensuring that patients are treated with dignity and their needs are met, optimizing the patient experience.
- Participates in systemic, interdisciplinary, and ongoing evaluation of programs, process improvement, and desired client-centered outcomes.
- Utilizes critical thinking and sound clinical judgment and works autonomously to solve problems.
- Facilitates resolution to patient problems and concerns, ensuring appropriate service recovery.
- Teams with site supervisor to ensure cohesion during day to day operations.
- Collaborates with Nursing unit managers/team leaders on nursing problems and interpretation of hospital policies to ensure patient needs are met.
- Directs the team regarding discharge planning and utilization management.
- Coordinates activities with other patient care units, when needed
- Performs “Rounding with Purpose”
- Suggests or recommends changes in plan of care based on nursing interactions with patient/family
- Ongoing collaboration and interaction with Case Management and Social Work throughout the day.
