Summary:

The Senior Life Communities nursing coordinator will be responsible for resident life experience beginning with the evaluation process through move in and beyond. The nursing coordinator will be an RN who facilitates communication with all medical personnel, the clinical team and pharmacies. The nursing coordinator will meet with prospective residents prior to admission to determine appropriate placement in Assisted Living or Memory Care; if prospective resident is appropriate for assisted living placement, the nursing coordinator will be responsible for assigning the appropriate level of care. A service plan will be developed upon admission, reviewed annually and updated as needed with any change(s) in condition.
The nursing coordinator is a member of the clinical leadership team who is committed to leading the clinical team, providing individualized and resident-centered care, and who is focused on ensuring high quality and exceptional resident satisfaction outcomes. The nursing coordinator will be responsible for facilitating regular care meetings with resident and families to ensure resident needs are met; additionally, the nursing coordinator will conduct regular care meetings/huddles with the care team. The nursing coordinator will be responsible for evaluating the clinical team and all care rendered by the clinical team to each resident. Input into the annual evaluation of each clinical team member will be communicated to the nurse manager to whom they report. The nursing coordinator may be requested by the nurse manager to assume on-call duties at times throughout the year. At all times, the nursing coordinator will represent the organization and facility to all prospective residents, current residents, resident families, healthcare providers, and the community.
Experience/Education Requirements:
  • Registered nurse, BSN preferred.
  • Minimum of 3 years experience working with senior population and residents with memory related disorders preferred

License or Certification Requirements:

  • Current Illinois Registered Nurse License
  • Current CPR certification

Responsibilities:

  • A service plan will be developed on admission, reviewed annually and updated as needed with any change in condition.
  • Acts as a clinical resource for the nursing team and assists in covering for the nursing team on a shift by shift basis if necessary.
  • Communicates with residents, families, CNA's, nurses, third party caregivers/agencies, pharmacies and physicians as needed.
  • Keeps nurse manager informed of any issues pending.
  • Conducts resident rounds regularly with attention to care being delivered by the clinical team.
  • If care issues are identified, initiates education, counseling and/or corrective action as necessary.
  • Develop and maintain 3rd party caregiver list and ensure that documentation meets all AL requirements.
  • Follow up on nursing concerns for all residents.
  • Input into the annual evaluation of each clinical team member will be communicated to the nurse manager to whom they report.
  • Maintain compliance with all IDPH standards within in the Assisted Living and Shared Housing Act. Participate in quality initiatives.
  • Responsible for facilitating regular care meetings with resident and families to ensure resident needs are met.
  • Additionally, the nursing coordinator will conduct regular care meetings/huddles with the care team.
  • Review, investigate, and address adverse events through resident issue forms and Riverside incident reporting system as directed by the nurse manager.
  • Reviews the 24 Hour Report every scheduled shift and follows up on care issues which may necessitate changes to the service plan.
  • Serves as a nursing supervisor handling day to day staffing needs as requested by the nurse manager or in the absence of the nurse manager.
  • The nursing coordinator will meet with prospective residents prior to admission to determine appropriate placement in Assisted Living or Memory Care; if prospective resident is appropriate for assisted living placement, the nursing coordinator will be responsible for assigning the appropriate level of care.
  • Will assist with onboarding new staff and ongoing education of the nursing team.

Lifting of patients off their beds that are unable to move independently is done with assistive lifting equipment. Horizontal movement of patients to and from carts/beds who are unable to move independently is done with assistive lifting equipment. Lifting equipment - carry 25'-150' waist high (IV bags, patient trays). Carry charts 25'-150' waist high. Filled laundry bags are filled half full so as not to weigh over 35 lbs. Carry fire extinguishers up or down 3-4 flights of stairs and several yards in an emergency.
