Work in conjunction with Residential Manager to provide support, evaluation and treatment to individuals with developmental disabilities living in a residential setting. Responsible for case management duties for assigned caseload.

ESSENTIAL DUTIES AND RESPONSIBILITIES Include The Following
- Write monthly progress notes, Individual Support Plans, assessments and objectives for annual reviews, and all other paperwork related to caseload as requested by Residential Manager
- Attend and participate in all meetings and chart data collected from programs regarding individual(s).
- Assist with development of behavior plans or other intervention strategies.
- Instruct, supervise and evaluate individuals in all areas of community living as outlined in each support plan and record data on all instructional programs.
- Implement all behavior plan interventions as assigned. These behavioral interventions may include physical restraint.
- Maintain valid Driver’s License and provide transportation as required by program.
- Assist individuals in scheduling and attending medical appointments and appropriately document. This may include covering shifts off-site, any hospital stays (i.e., Boston).
- Administer medications, as required, in accordance with medication administration procedures.
- Assist and support individuals as needed with scheduling and attending social and leisure activities such as: family gatherings, religious celebrations, community events, and social gatherings.
- Maintain residence and yard in clean, orderly condition as outlined by Manager. This may include: vacuuming, dusting, cleaning floors, bathrooms, laundry, and keeping lawn clear of trash and debris. Also responsible for keeping steps, walks, paths and ramps clear.
- Actively assist and support individuals as needed with personal care responsibilities, may include: diet, exercise, meal preparation, bathing, shaving, dressing, toileting etc.
- Assist and support individuals as needed with physical direct care, may include: physical or mechanical transfer of individuals from wheelchairs to van, chair, or shower etc.
- Adhere to all established lines of communication, dress in a professional manner and represent a positive and proactive attitude with coworkers, families and referral agencies.
- Responsible for writing reports, including daily shift reports, and other required documentation utilizing professional and appropriate documentation skills.
- Attend all required meetings, and adhere to all program routines and the schedules and needs of individuals under supervision.
- Participate in on-the-job training and provide coaching to new co-workers as directed.
- Adhere to agency safety standards.
- Perform additional duties, or substitute for others as required, including other programs.
- Adhere to all RTR policies and procedures.
- Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs.
- High School Diploma or equivalent.
- Education related to a field such as: psychology, social work, special education, or related human service work preferred.
- Valid Driver’s License.
- Basic knowledge of Microsoft Word and the internet.
- Successful completion and maintenance of all state and agency mandated trainings, including medication administration.
- Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
