Position Purpose
The Care Manager is responsible for providing case management to clients and families involved in the Momentum Program for a brief transitional period, including but not limited to coordinating services in the community to support a successful discharge plan, attending appointments, accessing resources, managing wrap-around funds and communicating with community partners.
The Momentum Program provides support for individuals with severe and persistent mental illness (SPMI) in transitioning from a Colorado Mental Health Institute (CMHI) located in Pueblo and at Fort Logan, or from acute hospital settings, back into the community in a less restrictive setting with community supports and services. Momentum is a short-term program to provide support for long-term client success in the community in order to avoid re-hospitalization. It was created to address the Olmstead Act requirements and is funded through a contract with the Office of Behavioral Health, State of Colorado.
This is a remote position. The Care Manager works from home and manages all day to day activities from a home office. There is an expectation of working regular business hours Monday through Friday, regular client visits in the community, and accurate, timely documentation. There are occasional requirements to go into an office, depending on the Care Manager’s location in the state.
Essential Duties
- Ability to manage day-to-day duties in a remote work setting.
- Participate in direct work with clients who have been committed, certified, or are voluntarily receiving emergency stabilization behavioral health treatment services in a CMHI or acute hospital setting.
- Provide case management duties in accordance with company policies, applicable laws, and the Momentum contract to assist clients and communities in managing the transition from hospitalization to less restrictive alternatives, employing a wraparound services approach.
- Participate in regular supervision.
- Assist clients and families in learning how to access services, coordinating various services and funding sources, identifying and strengthening formal and informal community resources, and mobilizing helping networks.
- Identify and report to the Program Manager or Assistant Manager regarding team activities and issues that may positively and/or adversely influence the delivery of services under the contract.
- Coordinate continuity of care with CMHCs including therapy, case management and ACT teams, coordinate with other local providers such as primary care, and generally assist the client in acclimating back into the community.
- Work with the appropriate state and city departments, local and public housing agencies to apply for appropriate housing vouchers when needed, and support individuals in locating, applying for, and attaining a housing rental.
- Coordinate with clients and providers in accessing needed behavioral health and physical health care needs.
- Assist with benefits acquisition where appropriate, including state and federal programs such as SSDI, SSI, Medicare and Medicaid.
- Work with clients, families and/or guardians to identify and purchase goods/services not covered by insurance or another payer source that are specific to individualized needs.
- Complete all documentation as required including but not limited to client contacts, collateral contacts, case management interventions, care plans, behavioral health care visits, and physical health care visits.

Qualifications
MINIMUM QUALIFICATIONS
- Bachelor’s Degree in Social Work or a related field.
- 2-5 years of mental health direct care experience.
- Ability to complete required training as required by the State of Colorado.
- Must be willing and able to complete a background check per state hospital protocol
PREFERRED QUALIFICATIONS
- Master’s degree in social work, counseling, human services or other related field.
Driving Requirements
- Valid driver’s license
- Proof of motor vehicle insurance
- Personal vehicle in good operating condition for use during work, including transporting clients
- No major violations in the past three years.
- No more than two moving violations in the past three years
- Ability to meet and maintain agency driving requirements and operate agency vehicles
- Momentum and TSP Staff must include a Business Use policy on their personal automobile insurance.
- Drivers must upload proof of ongoing auto insurance every 6 months into Paycom/Relias.
- Drivers must carry Bodily Injury Liability Limits of $300,000 each person/$300,000 each occurrence. Additional liability limits are recommended.
- Any Accidents and/or damages while using a personal vehicle for work purposes shall be the sole responsibility of the employee. RMHS is not responsible for any damage incurred to the employee’s personal property.
