What are the responsibilities and job description for the MDS Coordinator position at Ryders Health Management?
Hours: Monday - Friday 8:00am - 4:00pm or 7:00am -3:00pm, On-call rotation
Locations:
RN/LPN required 40 Hour - Waterbury, CT
RN required 40 Hour - Newington, CT
RN required 40 Hour - Stratford, CT
· Conduct and coordinate the development and completion of the Minimum Data Set (MDS) for each resident in accordance with current rules, regulations, and guidelines that govern the resident assessment instrument. Electronically transmitting all data to the Center Medicare and Medicaid Services (CMS)state and federal laws.
· Collaborate with professional case managers for Managed and Medicare services such as providing concurrent reviews, share plans of care and coordinate discharge processes.
· Coordinate change of payor and insurance appeals processes as needed.
· Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the MDS and care plan.
· Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary.
· Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
· Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident’s discharge plan
