About Activate Care:

At Activate Care, we’re on a mission to improve health equity and drive improved health outcomes across the country. Our Community Care Record platform enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs. Path Assist is our tech-enabled community health worker program for HRSN utilizing an evidence-based, structured intervention. Our goal is simple: increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend. 

About the Role:

We are on a mission to change healthcare. We’re doing this with Social Determinants of Health (SDOH) software solution that helps Care Navigators share information and collaborate with Care Based Organizations (CBOs) to improve physical and social health and well-being.

Reporting to the Vice President of Product & Design, the Product Manager - SaaS will direct product development and ensure high return on investment (ROI.) You’ll translate ideas into strategy and features, and follow product development from start to finish.You will be responsible for ensuring our business and user needs are being met on our new strategic product initiatives. You are a detail-oriented problem solver with a passion for building great products. You will be a leader on the Product and Design team to oversee the execution of the product roadmap. 

Responsibilities: 

  • Deep knowledge of platform capabilities, workflows, and gaps
  • Developing product requirements and roadmaps
  • Coordinate with multi-disciplinary teams to prioritize user stories and work to be done.
  • Partnering with product owners and coordinating with engineering, marketing and other multi-disciplinary teams
  • Develop and implement product strategies consistent with company vision
  • Work with senior management to create product plans and roadmaps
  • Produce and review product requirements documents (PRD)
  • Ensure products and releases are launched correctly and on schedule
  • Work with our researchers, designers, and product team  to define and clarify product requirements
  • Maintain a product backlog based on the direction set by Product Strategy
  • Answer incoming questions about the product and its capabilities
  • Work closely with QA and Engineering on validation and acceptance testing
  • Other miscellaneous duties as assigned
