Safe Vision Family Program is seeking a dedicated Creole-speaking Case Manager to join our team in Manhattan and Bronx, NY. The ideal candidate will have experience working with youth and families from diverse backgrounds, particularly those who speak Creole.
As a Creole speaking Case Manager, you will play a crucial role in providing support and guidance to individuals in need.
**Duties:**
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide crisis management and intervention services as needed
- Coordinate with external agencies and resources to ensure clients receive appropriate services
- Monitor and evaluate clients' progress towards their goals
- Offer guidance on accessing services such as Section 8, Child welfare, and Behavioral health
- Utilize motivational interviewing techniques to empower clients
-Provide case management services to clients, including conducting assessments and developing service plans
- Coordinate and monitor services to ensure clients' needs are met
- Advocate for clients and assist with accessing resources and support services
- Fluency in Creole and English

**Requirements:**
