What are the responsibilities and job description for the Clinical Supervisor, Home Health position at Salem Medical Center?
Salem Medical Center is looking for a full time Clinical Supervisor to work with the Home Health division!
The Clinical Supervisor is responsibilities include:
- Coordinating the interdisciplinary services provided by the Home Health program.
- Assures ongoing quality of care and service to the patients’ and families’ needs. Assumes supervisory function for the effective operation of the Home Health program. Assumes responsibility for monitoring quality and cost effectiveness of services provided.
- Organizes, assigns, reviews and evaluates and supervises the clinical staff for the agency.
- Ensures monthly staffing and on-call schedules for all clinical staff.
- Ensures annual performance evaluations are completed on Case Managers and Home Health Aides.
- Accepts referrals, verifies authorizations and ensures timely assignment.
- Assures that all care provided is appropriate to the payor.
- Assures that all clinical staff are working within the guidelines and regulations set for the State Licenses.
- Assures that all care clinical staff are working within the guidelines and regulations set for the State Licenses.
- Orients clinical staff on policies, procedures and philosophy of the Home Care Department.
- Maintains documentation verifying all CEU programs.
- Initiates process to identify clinical staff with specific training needs.
- Triages calls from physicians, patients, families and clinical staff with follow through on problems/concerns.
- Participates in case conference and case record reviews.
- Participates in administrative on-call system.
- Demonstrates flexibility and commitment to meeting the needs of the agency by participating on committees or special projects.
- Performs all other duties and responsibilities as necessary.
- Oversees Salem Home Health's ongoing Quality Assessment and Performance Improvement (QAPI) program.
Education: Baccalaureate of science preferred, Associates degree in nursing or diploma from a credible school of nursing.
Experience: At least two (2) years of progressive management experience and at least two (2) years of Home Health or Community nursing. Must have one year of OASIS scrubbing experience
License: Registered Nurse (RN) in the state of New Jersey.
