Job Responsibilities for a RN Clinical Service Coordinator includes:
- Responsible for coordination and implementation of the referral process and the coordination and initiation of the admissions process
- Works collaboratively with the Nurse Manager to ensure optimal hospice care
- Accountable for ensuring mission values are translated and integrated into daily work performance and relationships being guided by the values of compassion, dignity, respect, empowerment and growth
- Utilizes excellent interpersonal skills to promote both external and internal customer satisfaction
- Functions as resource person, mentor and coach for clinical staff
- Manages resources to provide quality delivery of care within cost-effective parameters and regulatory standards
- Plans daily work schedules for per diem and nurse liaisons
- Manages phone calls and communications with other members of the health care team
- Provides back-up to staff as appropriate
- Works in Referral Center every other Saturday and Sunday from 8:30a-4:30p
- Takes new pt referrals and inputs their information into Netsmart (EMR).
- Needs great communication skills and able to multi task.
