My name is Naomi, and I’m working with a Healthcare group in Naperville, IL. We are looking for an LPN or RN to be our full-time MDS Nurse Coordinator at our Transitional Care and Post-Acute facility.
Job Summary:
- Ensure current care plans and MDS for all residents.
- Ensure that all documents pertaining to the plan of care are consistent in accordance with current Federal, State, and Local standards, guidelines and regulations, facility policies, and those that may be directed by the Administrator.
Compensation: $70k-$85k (DOE)
Schedule:
- Mon-Fri in the facility and PRN weekend coverage on the floor.
Qualifications:
- Current, unencumbered license to practice as an LPN or RN in Illinois.
- Current CPR certificate or become certified within 90 days of employment.
- Experience with MDS/ MDS Certification.
- Working knowledge of OBRA, IDPH, and IDPA (for homes in Illinois) and standards and methods of documenting according to those standards.
