Job Description
Job Title: Urology Physician
Based at : Mountain Home , TN , USA

· Our Company requires Board Certified / Board Eligible Urology Physicians Services .
Requirements:
1.1.1. License –* physician(s) *assigned to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia)
1.1.2. when services are performed onsite All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor’s physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
1.1.3. Board Certification - physician(s) shall be Board Certified by the American Board of Urology (ABU), and be currently certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) obtained through the American Heart Association.
1.1.4. Credentialing and Privileging - Credentialing and privileging is to be done in accordance with the provisions of VHA Handbook 1100.19 .
1.1.5. physician(s) shall possess the requisite credentials enabling the granting of privileges.
1.1.6. Technical Proficiency - physician(s) shall be technically proficient in the skills necessary to fulfill the requirements, (referenced herein) that govern medical staff behavior.
1.1.7. Physician(s) shall be proficient in performing basic urologic evaluation and treatment including, but not limited to the care of BPH, kidney stones, male genital infectious diseases, bladder cancer, prostate cancer, kidney cancer, urinary incontinence and lower urinary tract symptoms and urinary retention. Proficiency shall include the surgical management of the above disease states.
1.1.8. Continuing Medical Education (CME)/Certified Education Unit (CEU) Requirements: Physician’s shall provide copies of current CMEs as required or requested .
1.1.9. physician(s) registered or certified by national/medical associations shall continue to meet the minimum standards for CME to remain current.
1.1.10. Training (ACLS, BLS, CPRS and VA MANDATORY):
physician(s) shall meet all VA educational requirements and mandatory course requirements defined herein; all training must be completed by the physician(s) as required .
