Madison Health and Rehabilitation is seeking a FULLTIME MDS Nurse to join our team
SUMMARY
Responsible for coordinating clinical assessment data on residents, putting this data together, and participating in formulating the Resident Care Plan
ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs assessments on residents using MDS and Medicare guidelines Ensures other departments have completed their assessment of residents within the allotted time frame and that they have signed the assessment form.
Coordinating the participation of gathering and formulating of Resident Care plans as dictated by the results of the data.
Responsible for conducting the Resident Care Planning Session with staff, residents, and resident families, providing data when there is a significant change.
Assures continuity of information from assessment process to the care plan process.
Coordinates nurses and CNA’s to attend care plans to discuss the residents’ care.
Assists DON and ADON in ensuring care plans are used and followed.
Assists residents when needed.
Serve as a liaison to rehabilitation personnel in planning resident programs.
Qualifications
Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
Experience as an MDS Nurse strongly preferred
